Využíváme antiemetickou profylaxi v onkologii dostatečně?

Význam profylaxe chemoterapií indukované nevolnosti a zvracení (CINV) u onkologických pacientů léčených chemoterapií (CHT) připomnělo sympozium společnosti MagnaPharm CZ s. r. o. na Jihočeských onkologických dnech, které proběhly v dubnu 2026. Přednášky i prezentovaný průzkum mezi českými onkology ukázaly, že je tato profylaxe potřebná především u pacientů léčených režimy chemoterapie s vysokým nebo středním emetogenním potenciálem (HEC, MEC) a že se stoupající emetogenitou CHT stoupá i potřeba kombinované antianemické profylaxe. Její nasazení musí proběhnout včas, protože nejvýznamnějším rizikovým faktorem CINV je CINV v předchozích cyklech CHT. Používá se při kurativní i paliativní CHT. U pacientů léčených HEC/MEC lze s výhodou použít fixní kombinaci palonosetronu a netupitantu, která v jediné dávce při současném podávání dexametasonu prokázala účinnou 5denní profylaxi CINV i během opakovaných cyklů CHT a 5. den po podání CHT významně větší účinnost než 3denní režim aprepitantu a granisetronu s dexametasonem.
