The paper presents recent opinion on individual imaging methods in the diagnostics of multiple myeloma (MM), and its differentiation from monoclonal gammopathy of undetermined significance (MGUS). We assess the possibilities and difficulties in the differentiation of individual manifestations of myeloma bone disease using conventional radiography, computed tomography (CT), magnetic resonance imaging (including whole body MR), 99mTc-MIBI scintigraphy, and FDG-PET/CT which combines positron emission tomography and multidetector computed tomography. We describe the contribution of new imaging methods for MM stratification (Durie-Salmon Plus staging...

Registry of monoclonal gammopathies together with the programme CRAB represent currently two main projects of the Czech Myeloma Group. The purpose of this project is the prospective data analysis of monoclonal gammopathies patients in the region of the middle and also the east Europe including incidence of diseases, therapeutical modalities used, the treatment results and the most frequent adverse events of therapy. It is ambitious project which could help us to improve the care about patients with monoclonal gammopathies.

Bortezomib (Velcade) is a representative of class of medicines called proteasome inhibitors. This drug represents quite new and unique medical treatment of the patients with multiple myeloma (MM). This article presents a retrospective analysis of effectiveness and adverse effects in patients with multiple myeloma treated at reference centers of CMG in the Czech Republic. A total 424 patients with multiple myeloma treated with bortezomib from June 2004 to June 2010 were evaluated. The median follow-up was 12,4 months. The mean age of patients was 66 year (34,1–86,5) and majority of them were patients with relapsed multiple myeloma. Of...

and smoldering multiple myeloma (SMM) Monoclonal gammopathy of undeterminated significance (MGUS) and asymptomatic form of myeloma are associated with a long-term risk of progression to active disease symptomatic multiple myeloma or related malignancy. As far as the malignant transformation of MGUS is concerned, size and type of the serum monoclonal immunoglobulin (monoclonal protein, M-protein, M-Ig), serum kappa and lambda free light chain ratio, number of clonal plasma cells in bone marrow, number of clonal plasma cells in peripheral blood, immunophenotype of plasma cells, and another factors seem to play a predictive role.

POEMS syndrome is a rare clinical entity with a very variable behaviour and clinical manifestation. It belongs to a group of diseases known as plasma cell dyscrasias and is characterised by a coincidence of monoclonal gammopathy with peripheral neuropathy and focal osteosclerotic bone lesions. Using three case reports, our paper presents different manifestations of this clinical entity as well as an assessment of possible diagnostic and therapeutic approaches to individuals with POEMS syndrome. The aim is to present this less known condition from the group of monoclonal gammopathies as well as the practical approach to diagnosing individuals...

Introduction: We present retrospective analysis of a group of patients with multiple myeloma treated in FN Hradec Králové from 1/2007 to 6/2010 by percutaneous vertebroplasty. Material and methods: A total of 18 vertebral bodies were treated in thoracolumbar spine region in 13 patients (3 men and 10 women), median age of 65 years. We watched the pain using the VAS (visual analoque scale), frequency of complications, functional outcome of patients (same, improved, worsened) and consumption of analgetics (increased, same, decreased). Patiens were treated at angiographic suit using transpedicular or posterolateral access. Results: Vertebroplasty...

The incidence of venous thromboembolism (VTE) is markedly increased in patients with multiple myeloma (MM); furthermore, the risk of VTE is increased during the induction phase of therapy, especially if a combination of thalidomide or lenalidomide with high dose dexamethasone or prednisone is used, while therapy containing bortezomib is not associated with risk of thrombotic complications. The effectiveness of pharmacological thromboprophylaxis using low-molecular-weight heparin (LMWH), acetylsalicylic acid (ASA) or warfarin has been demonstrated in numerous non-randomized comparative studies, and the results of one randomized trial are recently...

In addition to having specific effects in terms of glucose metabolism, the insulin-like growth factor type 1 (IGF-1) and insulin also possess a mitogenic effect that, by means of IGF1 receptors (IGF1-R) and insulin receptors (IR) often present on tumour cells, may be exerted by potentiation of tumour growth. The close link between carcinogenesis and glucose metabolism then results in interference of both these processes by drugs used to treat either condition. An understanding of the molecular mechanisms mediated by both receptors allows the clinician to predict adverse effects of treatment as well as to control the oncological safety of diabetes...

Author describes hormonal treatment of breast cancer pacients and review gynaecological care from prevention and diagnostics to monitoring of the first line hormonal treatment. Reasons of increasing breast cancer incidence are discussed and historical overview of hormonal treatment posibilities in pacients with breast cancer is summarized. The main part of the article characterizes influence of tamoxifen into woman organism, especially in her genital tract (endometrium changes) and side effects are analysed. The retrospective trial (Dept. Obst. and Gyn. 3-rd Medical Faculty) focusing pathological changes of endometrium under tamoxifen influence...

The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. Their prevalence increased from 1,169 to 2,565 survived cases between 1989–2005, with 3,430 cases expected in 2015. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly...

of colorectal carcinoma There might be big differences between the terms of targeted therapy and treatment tailoring. In our case report we describe situation when targeted therapy as the only method was associated with a clinical benefit in a patient with colorectal cancer. In addition to extrem importance of surgery also in treatment of metastatic disease this could be a clear documentation of need to explore the effect of targeted therapy in the majority of patients although we are often unable to define any predictive markers for it´s indication.

Hepatoblastoma is the most common primary hepatic malignant tumor in children. Adult hepatoblastoma is rare and the prognosis is poor. There are only case reports available for this diagnosis. We report a rare case of 26-year old man suffering from unresectable hepatoblastoma successfully treated by chemotherapy, transcatheter arterial chemoembolization (TACE) and other interventional – radiological techique for 11 years.
